A Clinical Study on Multiple Myeloma

Abstract
Twenty-five patients with multiple myeloma admitted to the Severance Hospital, Yonsei University College of Medicine between January 1965 and, August 1975 were studied, and the results obtained were as follows:
1. Forteen were male with male to female ratio of
1.3:1. The age distribution was between 40 and 66 with mean age of 54.7 years.
2. Nine cases had had the history of trauma on the sites of trauma on the sites of local bone pain and tenderness.
3. The mean duration from the onset of symptoms to diagnosis was about 2 years with range of 2 weeks to 3 years.
4. Clinical manifestations were as followings in frequency; bone pain (80%), hepatomegaly (32%), pallor(28%), anorexia (28%), G-I trouble (24%), headache (20%), fever (20%), weight loss(20%), dyspnea and palpitation (12%), masses (8%), epistaxis(8%), and retinal hemorrhage (4%). The locations of bone pain were on L-S spine in 48%, chest in 28%,neck in 12%, and extremities in 12%.
5. Hematologic findings revealed anemia in 80.0%, leukopenia in 24%, thrombocytopenia in 13.6%, and increased E.S.R. in 56.3%. ;

7. Twenty cases (80%) had the bony changes demonstrated by X-ray examination, which revealed on the skull and ribs in 48% respectively, on the spine in 40%, on the pelvic bones in 20% on the scapulars and clavicles in 12%, in each, on the humerus in 8%, and on the femur in 4%.
8. The total protein was increased in 57.1 % and reversed A/G ratio was noted in 61.9%.
9. Serum electrophoresis done in 23 case revealed M-component located on the fraction on alpha-2-globulin in 2(8.7%), on beta-globulin in (39.1%), and on gamma-globulin in 12(52.2% ).
10. The proteinuria was present in 23 out of 25(92. 0%). The Bence-Jones protein was positiveonly in 2 out of 18 (11.1%) by the routine thermal test, but 6 out of 16(37.5%) by urine electrophoresis, and the over-all incidence of Bence-Jones protein, was 44.4%.
11. Immunoelectrophoresis studies in 16 cases, showed; IgG-kappa in 6(37.5%), IgG-1ambda in 6(37.5%), Bence-Jones protein only in (12.6%), Fc ragment of IgG in the serum with lambda type of Bence-Jones protein in the urine in 1(6.3%), and the unidentified in 1(6.3%).
12. Complications during admission were neted in 6 cases(24.0%), and they were bronchopneumonia in 3, G-U tract infections in 2, and unknown infection in 1.
